I want to tell your story.

Neurodiversity questionnaire. 

(For however you define neurodiversity, mental health, processing, seeing musical notes as colors, ASD, ADD, AwhateverD.)

Please choose (at least) five of these questions to answer. If you are willing to include an image either of yourself or something that relates to your neurodiversity that would be excellent.
